Using Your Insurance
Flourishing Lives Counseling Center is in network with Blue Cross Blue Shield PPO and Blue Choice plans as well as Aetna PPO plans. Sometimes other health insurances have out-of-network benefits that you can utilize to cover your services here to some degree. The best way to find out what your cost would be using your insurance is to do a verification of benefits: call your specific plan and ask about how they would cover our particular services. We are happy to do this for you, and we recommend that you also call and double-check the same information.
When you call, the questions to ask are:
- Is this provider an in-network or out-of-network for me?
- What is my in-network and out-of-network deductible and how close are they to having been met?
- On what calendar date does my deductible year start over?
- After the deductible is met, what is my copay or coinsurance responsibility per therapy session (CPT code 90837)?
- Are there limits on the number of sessions per week or year, or the kinds of sessions or services covered?
- Is any approval or referral needed from my primary care physician?
Not Using Insurance
If you do not have insurance, or if you are choosing not to use your insurance, under the federal law known as the No Surprises Act, you have the right to receive a “Good Faith Estimate” explaining how much your medical care will cost. You have the right to receive a written Good Faith Estimate for the total expected cost of non-emergency items or services at least 1 business day before your scheduled service. If you receive a bill that is at least $400 more than your Good Faith Estimate for the same items/services, you can dispute the bill. Make sure you save a copy or picture of your Good Faith Estimate as it would be needed for the dispute process. For further information about your rights under this law, see www. cms.gov/nosurprises.

We accept cash, check and all major credit cards as forms of payment. Payment is due at the time of service.
